    目的:探究仿生物电刺激(BES)联合抗阻夹腿训练对产后盆底功能障碍性疾病(PFD)的改善作用。方法:随机数表法将本院2023年5月-2024年6月就诊的205例PFD患者分为BES组102例与联合组103例,均接受常规盆底肌训练(PFMT),BES组此外加以BES干预,联合组加以BES联合抗阻夹腿训练,均治疗8周。对比两组治疗前后盆底肌力分级、盆底功能电生理指标、尿动力学及压力性尿失禁(SUI)分度/盆腔器官脱垂(POP)程度。结果:治疗后,联合组盆底肌力分级、SUI分度及POP程度均显著优于BES组(P<0.05)。治疗后,联合组Ⅰ类/Ⅱ类肌纤维疲劳度、阴道动态压力与快肌最大肌电值分别为(﹣1.20±0.35)%、(﹣0.90±0.30)%、(76.30±5.51)cmH2O、(43.00±5.82)μV,均高于BES组的(﹣2.00±0.46)%、(﹣1.34±0.38)%、(68.85±5.76)cmH2O、(40.17±6.03)μV(P<0.05)。治疗后,两组尿动力学各指标均明显改善,且联合组优于BES组(P<0.05)。结论:BES联合抗阻夹腿训练能明显改善PFD患者盆底肌力、肌纤维疲劳度及尿动力学,进而减轻SUI/POP症状。

    Abstract:

    Objective To explore the effect of bionic electrical stimulation (BES) combined with resistance tightening leg training on postpartum pelvic floor dysfunction (PFD). Methods 205 PFD patients treated in our hospital from May 2023 to June 2024 were separated into BES group [n=102, pelvic floor muscle training (PFMT) + BES intervention) and the combined group (n=103, PFMT + BES + resistance tightening leg training) by random number table method, with 8 weeks of treatment. The classification of pelvic floor muscle strength, electrophysiological indexes of pelvic floor function, urodynamics and stress urinary incontinence (SUI) classification/ pelvic organ prolapse (POP) degree were compared between the two groups before and after treatment. Results After treatment, the pelvic floor muscle strength grading, SUI grading and POP degree in the combined group were significantly better than those in BES group (P < 0.05). After treatment, ClassⅠ/ ClassⅡmuscle fiber fatigue, vaginal dynamic pressure and maximal EMG value of fast muscle in the combined group were (-1.20±0.35) %, (-0.90±0.30) %, (76.30±5.51) cmH2O, (43.00±5.82) μV, respectively, which were higher than (-2.00±0.46) %, (-1.34±0.38) %, (68.85±5.76) cmH2O and (40.17±6.03) μV in BES group (P < 0.05). After treatment, the indexes of urodynamics in both groups were significantly improved, and the combined group was better than the BES group (P < 0.05). Conclusions BES combined with resistance tightening leg training can significantly improve pelvic floor muscle strength, muscle fiber fatigue and urodynamics in PFD patients, thus alleviating SUI/POP symptoms.
