    目的 利用拓扑优化确定个性化导板的布局,设计高承载能力导板。方法 以典型的下颌骨缺损模型为例,构建考虑骨骼与支架材料特性的下颌骨有限元模型。根据拓扑优化结果,设计了个性化导板。为了评估个性化导板的力学特性,模拟分析了常规导板与个性化导板固定条件下下颌骨、导板、螺钉的应力分布。结合Gibson-Ashby模型,设计弹性模量与皮质骨相当的多孔面心立方晶格结构,确定最终的支架方案。结果 个性化导板固定下,下颌骨、接骨板、螺钉的峰值应力分别55.86 MPa、291.1 MPa和122.53 MPa,分别比常规导板固定下降低了9.8%、32.0%和14.6%。结合个性化导板与多孔结构设计方案,得到了71.6%孔隙率的多孔支架三维模型。结论 本研究设计的个性化导板能显著降低了导板的峰值应力,提高了支架的可靠性。结合激光选区融化技术(SLM),可以快速制造出结构优异的个性化假体,以满足紧迫的时间需求。

    Abstract:

    The aim of this study was to determine the layout of the personalized plate with high bearing capacity by using topology optimization technology. Taking a typical defected mandible as an example, the finite model of mandible was constructed that considering the properties of bone and scaffold materials. To evaluate the mechanical properties of personalized plate, the stress distribution of mandible, guide and screws were simulated and analyzed under the under same occlusion compared to conventional plate. The results showed that the peak stresses of mandible, plate and screws of personalized plate fixation system were 55.86 MPa, 291.1 MPa and 122.53 MPa, respectively, which were 9.8%, 32.0% and 14.6% lower than those of conventional plate fixation system. Combined with Gibson-Ashby model of face centre cubic structure, the porous structure with elastic modulus equivalent to the cortical bone was designed, and the porosity was 71.6%. The personalized plate designed in this study significantly reduced the peak stress and improves the reliability of the scaffold. Combined with selective laser manufacturing (SLM), personalized prosthesis with excellent structure can be quickly manufactured to meet tight time requirements.
