    目的:建立颈动脉粥样硬化斑块流固耦合模型,研究模拟颈椎旋转手法拉伸对颈动脉粥样硬化斑块和颈动脉血管腔的生物力学变化影响,预测评估颈椎旋转手法对颈动脉粥样硬化斑块的破裂风险。方法:采用流固耦合模型模拟颈椎旋转手法的斑块和管腔的拉伸形变7%±9%。记录斑块和管腔的血流最大剪切力(FMSS)、最大壁切应力 (WSS) 、最大斑块壁应力(PWS)、壁面拉应力 (VWTS)和壁面压强等力学参数。结果:在16%颈动脉拉伸形变下,斑块的Max_WSS值是40.54 Pa,此处斑块可能表皮损伤;Max_PWS值为66.16 kPa,远小于斑块破裂阈值;内部纤维帽Max_VWTS和最大应变(156.75 kPa/0.56)大于破裂应变范围,能达到断裂临界值;管腔Max_VTWS(1040.30 kPa)已接近中膜破裂阈值,可能会引起血管损伤。结论:颈椎旋转至生理极限位时的颈动脉拉伸可能造成斑块表皮组织破坏导致脱落,斑块内部会形成损伤、溃疡和出血以及血管损伤,影响斑块稳定性。在颈动脉粥样硬化斑块的颈部疾病患者中应谨慎进行颈椎旋转手法治疗,手法前对斑块进行有限元评估可能是一种有效的安全性筛查方法。

    Abstract:

    Objective: The aim of the study is to establish a fluid-structure interactive model of carotid atherosclerotic plaque to study the biomechanical effects under cervical rotatory manipulation. It could be used to evaluate the rupture risk of carotid atherosclerotic plaque under cervical rotatory manipulation. Methods: The fluid-structure interactive model was used to simulate the tensile deformation of the plaque and lumen under the cervical rotatory manipulation by 7%±9%.The maximum flow shear force (FMSS), maximum wall shear stress (WSS), maximum plaque wall stress (PWS), wall tensile stress (Von mises stress, VWTS) and wall pressure were recorded. Results: The Max_WSS value of plaque was 40.54 Pa under 16% carotid stretch deformation.Max_PWS value was 66.16kPa, far less than the threshold of plaque rupture.The fiber cap’s Max_VWTS and the maximum strain (156.75kPa /0.56) were larger than the fracture stress/strain range. The lumen Max_VTWS (1040.30 kPa) has approached the threshold of rupture of the medial membrane, which may cause vascular injury. Conclusion: Large carotid artery stretching when the cervical spine rotates to the end range of motion may cause damage to the epidermal tissue of the plaque, leading to abscission. Lesions, ulcers, bleeding and vascular damage may form inside the plaque, which would affect the stability of the plaque. Cervical rotatory manipulation should be performed cautiously in patients with cervical disease with carotid atherosclerotic plaque, the FSI assessment of plaques before manipulation may be an effective safety screening method.
