目的：探讨拟行单髁膝关节置换术(UKA) 和全膝关节置换术(TKA)的膝骨关节炎(KOA)患者术前膝关节本体感觉特征。方法：选取在积水潭医院矫形骨科就诊的单间室膝骨关节炎患者29名,15例将接受 UKA 治疗(UKA组) ,14例将接受 TKA 治疗(TKA组)。术前1~3天进行测试,对比两组患者膝关节学会评分(KSS)以及两组患者和10名健康对照组位置觉和运动觉特征。结果： UKA组和TKA组KSS评分有显著差异(p=0.03),位置觉和运动觉无差异；KOA患者的术侧腿和未手术侧腿位置觉和运动觉均无差异,对照组左腿和右腿的位置觉和运动觉无差异； UKA组和TKA组术侧腿的位置觉均与对照组无差异,运动觉有显著性差异(p<0.001,p<0.001)；与对照组相比,UKA组和TKA组患者未手术侧腿的运动觉(p<0.001,p=0.003)和在60°时位置觉(p=0.01,p=0.02)均有显著性差异。结论：与健康对照组相比,KOA患者术前的膝关节本体感觉明显减退,但UKA组与TKA组患者术前本体感觉特征相近。
Objective: To explore the characteristics of knee joint position sense and kinesthesia of knee osteoarthritis (KOA) patients before surgery undergoing unicondylar knee arthroplasty (UKA) and total knee arthroplasty (TKA). Methods: Twenty-nine single-compartment knee osteoarthritis patients in the orthopaedic department of Jishuitan Hospital were selected. 15 patients were treated with UKA treatment and 14 patients will be treated with TKA treatment. The test was performed 1 to 3 days before the operation, and the KSS scores (KSS) of the two groups of patients were compared, and the characteristics of joint position sense and kinesthesia of the two groups of patients and 10 healthy controls were compared. Results: Results: The KSS evaluation results showed that the clinical performance of the TKA group was significantly worse than that of the UKA group (p=0.03); the joint position sense test results showed that the surgical leg of the UKA group and the TKA group had no differences compared with the control group. Compared with the control group, the unoperated side leg had a significant difference at 60° (p=0.01, p=0.02); the kinesthesia test results showed that the operation side leg of the UKA group and the TKA group had obvious defects ( p<0.01, p=0.01). Compared with the control group, the unoperated unaffected leg was also significantly reduced (p<0.01, p<0.01); there was no difference in position sense and kinesthesia between the UKA group and the TKA group. Conclusions: Compared with healthy controls, the proprioception of the knee joint in KOA patients was significantly reduced before surgery, but the characteristics of proprioception in the UKA group and the TKA group were similar.
